Schulich Heart Research

Cardiac disease is the leading cause of death in North America. Although lifestyle modification and control of risk factors are important, diagnostic and therapeutic interventions, such as those created and tested at Sunnybrook Research Institute (SRI), play a major role in the treatment of cardiac disease.

Innovations at SRI’s Schulich heart research program have already led to significant new advances that are improving cardiac care, including the following:

Our scientists have studied the use and effectiveness of angioplasty and the socioeconomic factors related to heart disease. We are also leaders in the use of intravascular ultrasound.

We are developing and evaluating new treatments for ischemic heart disease, valvular diseases, high blood pressure, congestive heart failure and electrophysiological disorders such as arrhythmia. Because of our strong partnerships with industry, new discoveries made at SRI are transferred quickly into improvements in health care technology and practice to benefit our patients. Our cardiac scientists work closely with the Schulich Heart Centre clinical team .

The Schulich heart research program is fully affiliated with the University of Toronto and many of their graduate students and fellows, particularly from the cardiovascular sciences collaborative program and the medical biophysics program, conduct research under the supervision of our scientists on site.

Areas of focus

Sunnybrook Research Institute’s Schulich heart research program is a leader in cardiovascular research in four major research areas:

  1. Interventions: including emerging approaches in cardiac surgery, endovascular treatment and medical management
  2. Imaging strategies: for disease assessment, treatment planning, therapy guidance and follow-up
  3. Outcomes and population health studies: health care outcomes research
  4. Cell biology: for regenerative therapies

  • Ongoing development and evaluation of cardiac surgery methods including:
    • alternative conduits in coronary artery bypass grafts and alternative heart valve implants
    • reducing the trauma associated with intervention, growing from early work on myocardial protection and continuing with evolving minimally invasive approaches to bypass and valve replacement procedures (Fremes)
  • Development and testing of imaging methods for assessment during interventions including optical fluorescence imaging of grafts, intravascular ultrasound of the arterial wall during angioplasty, and ultrasound studies of valve repair (Burns, Fremes, Wright)
  • Emerging focus on stem cell therapies for myocardial injury, including:
  • basic studies of chemoattractants for stem cells to damaged regions
  • MR-guided delivery of stem cells to infarct borders imaging assessment of therapy outcome
  • Evaluation of defibrillators and pacemakers for treatment of electrophysiological pathologies; emerging work in less invasive electrophysiological mapping and ablative approaches to better understand and manage complex arrhythmias and conduction problems (Crystal, Lau)
  • Studies in pharmacological management for hypertension and improved methods for blood pressure monitoring of hypertension patients (Myers)
  • Ultrasound methods development to exploit microbubble contrast for characterization of hemodynamics and myocardial perfusion (Burns)
  • Cardiovascular magnetic resonance imaging (MRI) research characterizing cardiac pathophysiology through oxygen-sensitive imaging in the deep veins and myocardium and through real-time acquisition methods to capture cardiac dynamics; improving noninvasive MRI strategies for mapping peripheral, neural and coronary vasculature and cardiac morphology (Wright)
  • Development of integrated information systems and visualization tools covering multiple imaging modalities for cardiac disease assessment and therapy guidance (in association with the Ontario Consortium for Cardiac Imaging , a multi-institutional effort led by Sunnybrook) (Burns, Wright)

Microbubbles for ultrasound

Application of microbubble technology for ultrasound imaging of the heart

Figure 1: Encapsulated microbubbles of gas used as a new contrast agent for ultrasound. They are smaller than the red blood cells, which are seen surrounding them. Work at Sunnybrook Research Institute (SRI) has developed a new acoustic imaging method that can detect these bubbles with high sensitivity, while suppressing the appearance of surrounding tissues, thus demonstrating, noninvasively, the perfusion of tissue by blood. This method, known as pulse inversion imaging, has had a significant clinical impact.
Figure 2: An area in the muscle of the heart. Microscopic vessels are important in this area because they are critically affected by a heart attack. Pulse inversion imaging with microbubbles has produced the first real-time (moving) images of perfusion of the myocardium. These images are now available to cardiologists at Sunnybrook as well as many other centres worldwide.
Figure 3: Pulse inversion imaging is capable of exploiting another nonlinear property of tissue and in doing so, improves the quality of conventional ultrasound images of moving structures such as the valve of the heart. On the left, the structure of the heart is covered in a kind of haze, which commonly limits the utility of ultrasound examination. On the right, however, the pulse inversion image clearly shows the leaflets of the mitral valve. Pulse inversion imaging was developed by a researcher and a graduate student in Sunnybrook Research Institute’s imaging research group and licensed by Sunnybrook to the ultrasound industry in 1999.

Integration of X-ray flat panel into an MRI system. Located at Stanford, this system is a collaboration between Dr. John Rowlands of Sunnybrook and Drs. Fahrig and Pelc of Stanford.
  • Development of registries for tracking outcomes post-infarct in hospitals (in cooperation with the Cardiac Care Network )
  • Prospective studies of the relationship between socioeconomic status and cardiovascular diseases; design of new approaches to assessing the utility of diagnostic tests (in cooperation with the Institute for Clinical Evaluative Sciences )
  • Studies examining how to improve the transition of care and outcomes of patients with cardiovascular disease, toward closing the gap in the quality of care received by these patients during transitions; and how to improve the quality of care and outcomes of people with atrial fibrillation (Ko)
  • Identifying and characterizing stem cells and genes involved in myocardial and vascular development
  • The focus of this effort is ultimately tissue regeneration for treatment of cardiovascular diseases

The Schulich Heart Program has earned an international reputation for pioneering some of the world’s most innovative ways to treat the heart and damaged blood vessels. We give the most vulnerable cardiac patients a new chance at life.

Critically ill patients are being treated with advanced technology to decrease complications, improve long-term outcomes and reduce treatment times. Patients too frail to undergo open heart surgery now receive novel minimally invasive procedures introduced through the generosity of donors or made accessible thanks to donors.
